Application Form
General Information
Under Article 11 of the Personal Data Protection Law No. 6698 (“KVKK”), data subjects (“Applicant”) who are defined as relevant persons have been granted certain rights regarding the processing of their personal data.
Pursuant to Article 13, paragraph 1 of the KVKK, applications regarding these rights must be submitted to our Company, which is the data controller, in writing or through other methods determined by the Personal Data Protection Board (“Board”). In this context, written applications to our Company must be made by filling out this form and:
- By registered mail with return receipt,
- Through notary public,
- By signing this form with a “secure electronic signature” as defined in Electronic Signature Law No. 5070 and sending it to our Company’s email address.
